FOR RETAIL HEALTH LICENSES:

      The South Plainffield Health Department's annual licenses are due by March 31st every year.  Kindly complete the relevant application form and include a check payable to the Borough of South Plainfield with the required fee. Applications received after the March 31st deadline will result in a $50.00 PER MONTH late penalty imposed.

     Applications can be found under "Quick Links" on the left corner of this page. Please do not leave any information blank on your application. Mail application with the fee to:  South Plainfield Health Department, 2480 Plainfield Avenue, South Plainfield, NJ 07080

     Please note that there will be an additional $150.00 added to the license fee total for failure to attend a 2020 Food Handler Safety Course.  For questions in this regard you may contact the Health Officer at 908-226-7634.

FOR PET LICENSES:

    Pet licensing is handled through the Muncipal Clerk's Office.  Contact 908-226-7607.  Please be advised if you no longer have a pet to contact us and we will remove your pet from our system so that you will not receive automatic late notices.  Please see the links below for pet licensing applications. 

FOR VITAL STATISTICS; BIRTH, MARRIAGE DEATH:

    Vital statistics are handled through the Municpal Clerk's Office. Contact 908-226-7607.  Please be advised all vital statistic forms and directions can be found under the "forms" tab on the Home Page, or under the link called "FORMS" , then scroll down and you will see Registrar Forms.   

DIVORCE DECREES

New Jersey divorce decrees are available through the Superior Court of New Jersey Records Center.
